{"id":4739,"date":"2023-09-25T03:33:35","date_gmt":"2023-09-25T06:33:35","guid":{"rendered":"https:\/\/4tuneagencia.digital\/blog\/colorectal-cancer-can-ai-be-used-for-a-colonoscopy-medical-news-today\/"},"modified":"2023-09-25T03:33:35","modified_gmt":"2023-09-25T06:33:35","slug":"colorectal-cancer-can-ai-be-used-for-a-colonoscopy-medical-news-today","status":"publish","type":"post","link":"https:\/\/4tuneagencia.digital\/blog\/colorectal-cancer-can-ai-be-used-for-a-colonoscopy-medical-news-today\/","title":{"rendered":"Colorectal cancer: Can AI be used for a colonoscopy? &#8211; Medical News Today"},"content":{"rendered":"<p>Two articles published today in the <em>Annals of Internal Medicine<\/em> journal examine the benefits and limitations of using <a rel=\"noopener noreferrer\" href=\"https:\/\/www.sas.com\/en_us\/insights\/analytics\/what-is-artificial-intelligence.html#:~:text=Artificial%20intelligence%20(AI)%20makes%20it,learning%20and%20natural%20language%20processing.\" target=\"_blank\" class=\"content-link css-1pg8eb5\">artificial intelligence<\/a> with <a href=\"\/articles\/colonoscopy\" class=\"content-link css-1pg8eb5\">colonoscopy<\/a> procedures.<br \/>The results of these studies seem to contradict one another with one showing no diagnostic improvements and the other finding a reduction in the missed rate of colonoscopies.<br \/>\u201cOverall, the two studies are different in design but come to contradictory conclusions regarding the use of <hl-trusted-source source=\"BMC\/BioMed Central\" rationale=\"Peer reviewed journal\"><a rel=\"noopener noreferrer\" href=\"https:\/\/biomedical-engineering-online.biomedcentral.com\/articles\/10.1186\/s12938-015-0120-7#:~:text=06%20January%202016-,Computer%2Daided%20detection%20(CADe)%20and%20diagnosis%20(CADx),cancer%20with%20likelihood%20of%20malignancy\" target=\"_blank\" class=\"content-link css-1pg8eb5\">CADe<\/a><\/hl-trusted-source> (computer-aided detection) in colonoscopy,\u201d said <a rel=\"noopener noreferrer\" href=\"https:\/\/nyulangone.org\/doctors\/1073529871\/aasma-shaukat\" target=\"_blank\" class=\"content-link css-1pg8eb5\">Dr. Aasma Shaukat<\/a>, a gastroenterologist and director of outcomes research for the division of gastroenterology and hepatology at NYU Langone Health in New York who was not involved in the studies.<br \/><strong>\u201cOne thing the two studies agree on is that detection of non-neoplastic findings (<a href=\"\/articles\/322428\" class=\"content-link css-1pg8eb5\">polyps<\/a> that are not of worrisome histology) do increase with CADe,\u201d Dr. Shaukat told <em>Medical News Today<\/em>.<\/strong><br \/>Artificial intelligence (AI) is used extensively in medical sciences, according to a <hl-trusted-source source=\"PubMed Central\" rationale=\"Highly respected database from the National Institutes of Health\"><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7640807\/#:~:text=Artificially%20intelligent%20computer%20systems%20are,prescriptions%2C%20and%20remotely%20treating%20patients.\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-1pg8eb5\">2020 review on the topic<\/a><\/hl-trusted-source>. Applications that diagnose diseases, research end-to-end drug discovery and development, improve communication between physician and patient, transcribe medical documents, and remotely treat patients often use AI.<br \/>\u201cArtificial Intelligence is rapidly integrating into every aspect of our daily lives,\u201d said <a rel=\"noopener noreferrer\" href=\"https:\/\/www.providence.org\/doctors\/gastroenterology\/ca\/orange\/james-lee-1750358735\" target=\"_blank\" class=\"content-link css-1pg8eb5\">Dr. James Lee<\/a>, a gastroenterologist with Providence St. Joseph Hospital in Orange, CA, who was not involved in the studies.<br \/>\u201cThe role of AI in colonoscopy detection of colon polyps will be continually evaluated and advanced in coming years,\u201d he told <em>MNT<\/em>. \u201cCurrently, the role of AI during the colonoscopy is to detect polyps on the screen by placing a green box (GI-Genius, Medtronic), attracting the endoscopist\u2019s attention in real-time.\u201d<br \/>There are some risks to colonoscopies, no matter which kind. These include having the doctor take extra tissue around the polyp and an increased risk of bleeding.<br \/>\u201cIn addition, the cost can increase by 15%,\u201d <a rel=\"noopener noreferrer\" href=\"https:\/\/www.memorialcare.org\/providers\/ashkan-farhadi\" target=\"_blank\" class=\"content-link css-1pg8eb5\">Dr. Ashkan Farhadi<\/a>, a gastroenterologist at MemorialCare Orange Coast Medical Center in California who was not involved in the studies, told <em>MNT<\/em> that costs can increase using this type of technology.<br \/>The <a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M22-2619\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-1pg8eb5\">first study<\/a> did not find improved detection of premalignant or malignant lesions of <a href=\"\/articles\/155598\" class=\"content-link css-1pg8eb5\">colorectal cancer.<\/a><br \/>For this study, more than 3,000 people with a <a href=\"\/articles\/fit-test\" class=\"content-link css-1pg8eb5\">positive fecal immunochemical test<\/a> (FIT) were randomly assigned to receive colonoscopies with or without (computer assistance) for the detection of advanced colorectal neoplasia, adenomas, serrated polyps, and non-polyploid and right-sided lesions.<br \/>The researchers said they chose people with positive tests because they have the highest prevalence of advanced colorectal neoplasias. Therefore, they provide the best context for investigating computer-aided detection and diagnosis of colorectal cancer.<br \/><strong>Although the scientists observed a small effect, they did not find significant differences between the two groups in the detection rate of advanced colorectal neoplasias or in the average number of advanced colorectal neoplasias that were detected per colonoscopy.<\/strong><br \/>They said the findings suggest additional research and more defined detection parameters in CAD before it can be integrated into routine care.<br \/>\u201cThe limitations of study one is that all participants had <a href=\"\/articles\/occult-blood-in-stool\" class=\"content-link css-1pg8eb5\">occult blood in their stool<\/a>, and the study design was only to detect differences in advanced adenomas. The detection rate in the control group was 62 percent, and 64% CAD assisted colonoscopies. The baseline of 62 percent is very high when compared to regular screening colonoscopies this rate varies from 7 percent to 55 percent. This could be the reason why there was no difference between advanced adenomas.\u201d<br \/>\u2014 Dr. James Lee, gastroenterologist<br \/>In the <a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M22-3678\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-1pg8eb5\">second study<\/a>, researchers from Humanitas University conducted a systematic review and meta-analysis of 21 randomized trials with 18,232 participants on using CAD with a colonoscopy.<br \/><strong>The researchers reported a higher detection rate of polyps but not for adenomas with a higher risk of cancer progression. They also found a 55% reduction in the miss rate when detecting adenomas.<\/strong><br \/>However, they noted that the AI-assisted procedures could increase over-diagnosis and over-treatment of non-neoplastic polyps. They can also increase the amount of time to complete a colonoscopy.<br \/>\u201cI think this study is more reliable because it pulled data from a large number of studies and a high number of participants were included,\u201d Dr. Farhadi said.<br \/>The study mostly involved gastroenterology experts and researchers said it could be more helpful to less experienced doctors.<br \/>\u201cBoth studies showed similar findings that AI did not improve the detection of so-called advanced adenomas,\u201d Dr. Lee said. <br \/>\u201cStudy two showed that the AI did increase overall detection and decreased the miss rate. Still, the harms of this study showed increased inspection time and increased number of <a href=\"\/articles\/319757\" class=\"content-link css-1pg8eb5\">polypectomies<\/a> for non-neoplastic lesions.\u201d<br \/><strong>\u201c<\/strong>The accuracy of a colonoscopy depends greatly on the operator,\u201d Dr. Farhadi said. \u201cOperators who typically find a low to medium number of polyps would benefit from using AI. Physicians who are highly skilled and performing colonoscopy probably won\u2019t need AI.\u201d<br \/>\u201cI believe that AI is only as good as the endoscopist,\u201d Dr. Lee said. <br \/>\u201cIf AI does not see the part of the colon (parts behind the fold of the colon because the endoscopist failed to expose that part of the colon), then AI has no idea that there are polyps there. Hopefully, in the future, if AI can notify endoscopists whether a part of the colon was not visualized or missed, then we probably have more complete assistance from the AI and more meaningful data.\u201d<br \/><strong>Whether using AI or not, experts say it is essential to ensure colonoscopies are accurate.<\/strong><br \/>\u201cI think it\u2019s important to use AI for colonoscopies, Dr. Shaukat said, \u201cbut it is not essential if the endoscopist is doing other things to ensure a high quality exam.\u201d<br \/>\u201cWith AI, one thing to watch is that it should not increase the non-neoplastic tissue removal, which adds cost and no benefit. Future enhancements of AI are awaited that can predict the histology of the polyp based on its surface, avoiding unnecessary tissue removal.\u201d<br \/>\u2014 Dr. Aasma Shaukat, gastroenterologist<br \/>The studies did not answer the most crucial question, Dr. Lee noted.
